Job Summary

Summer Program Preparation and Integration
• Collaborates with Program Manager and Summer Program Coordinator to ensure that each student is appropriately placed on a trip that will challenge them and set them up for success.
• Works with each student who has passed the 4 week self-selection period to ensure that they are physically and emotionally ready for both their first and second trips; works with each student to celebrate successes on their trips, reflect on learning opportunities and reintegrate their experiences through writing essays, weekly mentoring conversations and group interactions with fellow students.

Program Data Tracking
• Enters and tracks data accurately on a daily basis, for now in Raiser’s Edge donor database and then in Efforts to Outcomes student database, so data can be used to assess student progress, staff evaluations, and organizational learning.
• Works with Program Manager and Senior Mentors to consistently assess and measure own results of program activities.
• Utilizes data tracking and analysis as a learning opportunity, including oral presentations of student assessments and observations.

Administrative
• Assist with general administrative duties including answering phones, processing mail, routing information, program logistics and events, and filing.

Minimum Qualification Requirements
• A Bachelor’s degree
• 3-5 years of work experience after college
• Previous work with adolescents, teaching or non-profit experience a plus.
• Demonstrated excellence in interpersonal, oral, and written communication skills
• Excellent organizational and project management skills

Title: Stabilization Case Manager - Bilingual - Homeless Prevention and Rapid Re-Housing
Reports To: Crisis Center Program Manager

Expected Goals and Objectives

Goal One: To assist clients in obtaining stable, safe, affordable housing by:
---Working consistently within LCN’s mission, vision, philosophy of service and team approach.
---Performing case management assessments to identify clients’ service needs, strengths and resources.
---Assessing client’s eligibility for short or medium-term financial assistance.
---Developing and implementing service plan with client outlining clear goals and tasks along path to clients’ housing stability and financial self-sufficiency.
---Requesting rental and utility assistance payments on behalf of client; conducting reassessment if client is in need of additional financial assistance.
---Utilize clinical skills and knowledge to guide case management sessions and to intervene or refer as appropriate.
---Providing clients with the motivation, support and encouragement needed to empower themselves.
---Counseling and mentoring clients in both individual and group settings.
---Delivering life skills training through both formal and informal techniques to address important skills such as conflict resolution, financial management and budgeting, health and nutrition, mental and emotional health, safe sex, etc.
---Provide quality internal referrals to Housing Locator, Housing Inspector/Follow-Up Specialist, Placement Manager, and Curriculum Specialist.

---Maintaining excellent and open communication with clients and following-up with clients on a regular basis
---Documenting all services in a timely and accurate manner.
---Utilizing Homeless Management Information Systems database to enter client information and to document services.

Goal Two: To connect clients to community resources:
---Conduct client advocacy via in-person, phone, and/or letter for various participant needs; conduct advocacy to third party public benefit and service providers such as IDHS, Social Security, doctors, schools, etc.
---Referring clients to other supportive services as needed, including housing, food pantries, clothing closets, legal aid services, substance abuse and mental health counseling.; coordinating these services as needed to obtain wrap-around support for clients.
---Educating clients regarding positive uses of financial institutions & credit, payday loan alternatives, and credit repair services.

In addition to the responsibilities listed above, all La Casa Norte employees are responsible for contributing to the overall success of La Casa Norte by:
---Attending weekly staff meetings and other staff trainings.
---Participating in La Casa Norte’s fundraising events.

The EMR Manager will also support PIH's Lesotho-based IT team and may take on additional duties as the program requires, including oversight of data entry and management activities.

Project Coordinator – Institute for Health and Social Justice

Overall Responsibilities: The Project Coordinator for the IHSJ will be based in Boston and will support the Director of the IHSJ, the Policy and Advocacy Director, and Research and Administrative Assistant by tracking legislative activities related to Haiti, food security, global health funding and health systems strengthening. S/he will collaborate with Development and Communications’ teams on outreach to increase membership for PIH’s on-line community. Assume primary responsibility for scholarly documentation and manuscript preparation.
